To submit a Heart Warrior or Heart Family request for support,​
one or more of the child's support team from the list below
must contact us using the Submit Heart Family Request button.
Requests for support must include:
- Name and credentials of requester
- Main diagnosis of child
- Child's age & gender
- How KHT can best support the child and/or family
Submissions do not need to include the name of the child or family.
Child's Support Team
Social Worker
This social worker can be from the hospital, clinic, school, or county/state. Please ensure that this person knows the child's diagnosis so KHT can provide appropriate support.
Nurse Practitioner or
Physician's Assistant
This Nurse Practitioner or Physician's Assistant can be from the pediatric cardiac clinic or from the NICU or PICU the child is currently in or will be admitted to.
Child Life Specialist
Most pediatric intensive care units (PICUs) have Child Life Specialists on staff to help children and families navigate the process of illness, injury, disability, trauma, or hospitalization.
Cardiologist / Surgeon
The child's cardiologist or cardiothoracic surgeon can reach out to KHT to request support for the family.
Nurse
This nurse can be from the pediatric cardiac clinic or from the NICU or PICU the child is currently in or will be admitted to.
Employer of Caregiver *
* This request must be in addition to a request from another person from this list.
Employer must provide proof of employment of caregiver and why they are requesting support for family.
